Registration Information
Please contact the school that your child will be attending to set up an appointment for registration.Please bring the following with you to your appointment:

Immunization Records- dates

Immunization records are part of the paperwork requested from your child’s previous school. If they are not included, it becomes the responsibility of the parent, guardian, or custodian to provide this information.
2. Custody papers, if needed.
3. Former School Information (name,address,grade)
4. If special education, a copy of current IEP and/or MFE is special services are required.
5. Emergency Information- Who to contact in case of an emergency.
6. Proof of residency- Two (2) of the following:

-Income tax statement showing you have paid either Sheffield Village or Sheffield Lakes City taxes. |
|
|
-Utility bill |
|
|
-Voter's registration |
|
|
-Purchase or lease agreement |
|
|
-Signed rent receipt |
7. Social Security Card - Childs Social Security Number
8. Child's Birth Certificate.
NOTE: Failure to provide all these items will delay the registration process.
