Help Me Grow to PreSchool Transition Process

The Help Me Grow Service Coordinator may invite the school psychologist and/or district representative to attend a Transition Planning Conference to discuss transition from early intervention services to preschool for a child suspected of having a disability.  This typically occurs approximately six months before the child's third birthday, but may occur up to nine months before the child's third birthday.

If there is a suspected disability, the school psychologist will prepare appropriate paperwork for the team to plan an evaluation.  The school psychologist and/or district representative will also provide information about possible service delivery options if the child should be eligible for services.  The school psychologist and/or district representative will conduct an observation of the child in his/her home environment at this time.

Additional time is scheduled for further evaluation activities to occur.  If the child is determined to be eligible for special education and related services as a preschool child, the school district will work with the family to ensure that an Individualized Education Program (IEP) is in place and implemented by the child's third birthday.
